Abstract |
Background. The article is devoted to the dynamic analysis of the social structure of medical staff and identification of the factors influencing the formation of groups of urban hospitals. The importance of the problem is defined by the need to achieve the goals, formed in the Concept of Health of the Russian Federation up to 2020, and as a result, the need for a new approach to staffing of healthcare organizations. The aim of the study was to define the factors influencing the formation of physician groups, based on the dynamic study of the social structure of the staff of city hospitals.
Materials and methods. To achieve this goal the authors conducted the study of the structure of medical staff by gender, age, length of time in the ordinary general hospitals of Nizhny Novgorod. The study was conducted on the basis of personnel services materials, 179 units of observation (doctors - employees of public hospitals) were studied.
Results. Investigation of the structure of the medical staff by age revealed the inequality in the distribution of physicians by the age group, there was a trend to reduce the proportion of male doctors in government hospitals. Age-specific structure of the city's hospitals at this stage is more optimal. Sharp reduction in the inflow of young doctors is alarming at the same time. The share of professionals in the 25-29 age group in 2012 was less than 5 ± 2,5%; p < 0,01 of all hospital doctors. In 2005, the structure of the medical staff of hospitals consisted of professionals employed less than ten years ago, and the number of physicians has steadily decreased with increasing length of service in these health facilities. Provided the transition of results to the situation ten years later, the chart of distribution of physicians for the duration of work experience in these hospitals in 2012 to a large extent repeats the same in 2005, which indicates an increase in the stability of medical teams of the organizations studied. It should be noticed that the proportion of doctors, who have experience in these hospitals from 0 to 4 years, is more than twice of the proportion of physicians aged 25-29 years, i.e. over the past five years the hospitals hired experts with experience in health care. Results indicated that the lack of systematic work on staffing urban hospitals over the past two decades led to the formation of a significant gender disparities and uneven agestructure in consulting teams. Significant losses were associated with the outflow from the industry of cohorts of graduates of medical schools of early and mid 90s. It is the loss of this group of physicians for the healthcare that led to the formation of a "gap" in the group of 35-39 years in 2005 and the reduction in the proportion of physicians of 45-49 years in 2012. Activities which carried out in this industry over the past five years has led to a tendency to plan the optimization of the medical staff of the hospital in the aspect of age-structure and stability of teams. At the same time alarming is the decrease in the share of young physicians in the frame structure of hospitals.
